Overview
Tier Progression Logic is the backbone of Cedar & Iron’s flexible, responsive support model. It acknowledges that aging is dynamic—needs shift, emotional weight fluctuates, and family capacity evolves. This logic ensures clients receive the right level of stewardship—not too little, not too much—and that transitions between tiers are handled with clarity, consent, and care.
Purpose:
- Protect the integrity of the service—avoid overpromising or underdelivering.
- Support client dignity by adjusting with them, not for them.
- Relieve family advocates when signs of burnout or complexity arise.
- Enable sustainable service delivery—prevent your own overload.
Key Principles:
- Progression is need-based, not upselling.
- Fit is reassessed regularly, not just at crises.
- Consent and collaboration are required for all shifts.
- Ad hoc add-ons can buffer without full tier changes.
- Downgrades are welcome if stability is regained.
Tier Shift Workflow
Here’s how a client may move from one tier to the next, triggered by changes in need, capacity, or goals.
Peace → Care
Trigger Signs:
- You observe repeated minor gaps: clutter, appointment confusion, skipped routines.
- Family reports missed medications, increasing isolation, or emotional instability.
- Light support tasks are now spilling beyond the monthly cadence.
