Confidentiality Statement

All personal, family, and health-related information shared during visits is considered private and will not be disclosed unless:

• There is a clear safety concern that warrants escalation
• You give written or verbal permission to share
• Required by law (e.g. suspected abuse or danger to self/others)

Cedar & Iron will not share recordings, photos, or summaries without clear purpose and consent.

Family Updates

If the client consents, families may receive:

• Monthly visit summaries
• Mid-month check-in notes
• Urgent flags (decline, safety risks, etc.)
